If you have been ill for more than 7 days or if you know you will be off work for more than 7 days, you will need to speak to a doctor to request a sick note.

If you have been in hospital, the ward should issue this for you prior to discharge, please ensure you obtain this from them to reduce the burden on the GP.

Please contact the practice by telephone for an appointment if you need to get a new certificate.

A sick note can be backdated if it takes longer than the 7 days to get a suitable appointment.
